TIPS ON PREPARING FOR YOUR PET'S FIRST APPOINTMENT

The Questionnaire and Medical Records

Your completed questionnaire is a very important part of the first appointment, so please plan to take some time to thoroughly complete this document. It is sometimes helpful to read through it to get a sense of the information we’re looking for, and then go back and observe your pet. If you don’t know some answers that’s fine, but the more information you can provide, the better, because who knows your pet like you!

We also require that you provide a medical history (including vaccination history and recent lab work), from your “regular vet” or any specialists that your pet may have seen. You are responsible for contacting these practitioners and requesting that medical records be forwarded to our office.

Please plan to have the completed questionnaire and medical records arrive in our office at least 7 days prior to your appointment. This allows Dr. Sturm the opportunity to review the information you have provided so that she can prepare for your time together.

How We Schedule Appointments

Initial appointments are scheduled for an hour on Wednesdays, Thursdays or Fridays. They are scheduled for an hour to allow Dr. Sturm adequate time to: explore with you the information you have provided, make a thorough assessment of your pet’s medical condition, develop a plan for treatment with you, and, in many cases, to begin treatment.

Due to the preparation time involved prior to and during the first appointment, we only schedule one initial exam a day.

Follow-up appointments are scheduled for 30 minutes. These may be scheduled Wednesday - Saturday.

We encourage all of your pet’s caretakers to attend the initial visit, if possible. We have found that the entire family’s commitment to treatment contributes to a more positive experience.

Be sure to write down any questions you might have and bring them with you. You might also want to take notes about treatment plans and the reason(s) behind them.

We can answer general questions if you have more than one pet in your family. However, each pet must have an initial visit to address their particular concern(s).
